Please let us know what we do well and where we can improve our services.
Please assist us by completing the following form which will be emailed directly to management.
If you prefer, hard copies of the Feedback form are available in the Reception areas of both The Lodge and The Manor where completed forms can also be returned to. Completed forms can also be handed to a member of staff, Director of Care, General Manager Operations, General Manager Corporate Services or CEO. Alternatively, you can email the form to firstname.lastname@example.org.
